Page 2 contains
the new St Anastasia CYO Code of Conduct and the eligibility rules
for Non St
Anastasia Students.
Both sections on page
2 need to be signed/ initialed by a parent.
The Code of
Conduct also must be read and signed by the student athlete.

Online Form
- Can be completed as a Word Document and then
saved on, and printed from, your personal computer.
Print and Complete
- Print the form and then complete the needed information.
If you only need
